As you know, I decided to take a leave of absence effective May 28, 2011.  I am taking this short break in order to process some of the events that have taken place in my personal life over the past two years.  When I return, I believe that I will be better able to serve you.

I am maintaining my privileges at memorial Hospital and Clinics so that when I return to practice medicine in Colorado, I can get started immediately.

As always, you have the right to choose whomever you want to help you with your healthcare needs. I understand you may seek medical attention while I am on leave and you may need your medical records. If you want your medical records, please send a written request (dated and signed) with proof of ID (copy of driver's license or other identifying information) and a $15.00 copying fee to the following address: P.O. Box 15784, Colorado Springs, CO 80935-5784. Click here for the release form.

The medical board recommends 14.00 for the first page and .50 for each additional page. However, to simply this process , I only require a flat cost of $15.00. Also, include the name, phone number and fax number to the party that you would like your information sent to.
