Insurance:

We accept all insurances as well as Medicare and Medicaid. If you do not currently have insurance and do not think you qualify for Medicare or Medicaid, please call our office at 719.282.4066. We have access to resources that may be able to help you in your situation.

We accept most insurance providers. If you do not see your plan on the list below please contact our billing department at (719) 282-4066. Our staff will be happy to clarify your policy and benefits. As a courtesy to our patients we will file your secondary and tertiary claims. Be sure to bring your insurance cards to your appointment.

Benefits:

Knowing your policy benefits, deductibles, and co-payments will avoid any unexpected billing problems. For new OB patients, a meeting with our billing department where we will review your OB benefits will occur at your first visit. Many insurance companies have unique benefits during pregnancy. We will do our best to explain your policy; however, it is ultimately your responsibility to understand the coverage policy for your insurance carrier.

Referrals:

If your policy requires a referral it is your responsibility to obtain that referral for our billing department.

Tri-Care Prime Patients:

You must obtain a referral from your PCM before an appointment can be made.

Medical Release Forms

Caring OBGYN requires completion of a medical release form from clients in order to:

* Diagnose their medical/psychiatric/psychological condition
* Plan individual care and treatment
* Communicate with other health professionals concerning a patient's care
* Document services for payment/reimbursement
* Conduct routine health care operations

Pre-Registration Procedures

To facilitate the appointment process we offer pre-registration. Included on this page is a portable document format (PDF) form that can be downloaded and completed in advance of your appointment.

Upon completion, print the form out. Note that it cannot be saved and e-mailed. You may mail the completed form(s) to us or fax it (719) 282-4067. In this way we will have your information in advance and can be prepared from an administrative standpoint to serve you more efficiently.
