Pre-Registration Procedures

To facilitate the appointment process we offer pre-registration. Included on this page is a portable document format (PDF) form that can be downloaded and completed in advance of your appointment.

(Note: You will need Adobe Reader to complete the Pre-Registration form. If you do not have this application on your computer we've provide a link where you can down load it).

Upon completion, print the form out. Note that it cannot be saved and e-mailed. You may mail the completed form(s) to us or fax it (719) 282-4067). In this way we will have your information in advance and can be prepared from an administrative standpoint to serve you more efficiently.
